Check Your Financial Aid To-Do List to See Required Documents or Verification Items
- To check your Financial Aid status and required documents or verification items, visit SDCCD Student Portal.
- Log in using your credentials. This is the same portal where you enroll in classes.
- After logging in, click on "view financial aid". This will show you the current and relevant information regarding your Financial Aid.
- To determine which forms or verifications are needed to complete your file, and to view all notifications and emails, click on the "notifications" banner on the College Student Dashboard.
- If your file is complete and you want to see any Financial Aid awards you may be eligible for, select "View Your Financial Aid".
Submitting Forms & Documents

Electronic Submission of some documents PDF is preferred:
- Some forms can be submitted online.
- Download, fill out, and sign the necessary form from the provided links.
- Go to the 'Submit Form Online' link.
- On the submission page, choose 'Forms/Applications' then 'Financial Aid Form/Verification'.
- Ensure the form you select matches the attached PDF. Attach all necessary supporting documents.

Faxing Documents:
- Wait 24-48 hours for faxed documents to be processed and reflected on The Portal.
- If faxed on Thursday afternoon, updates may not appear until the following Tuesday. Check office hours for further clarity.
- Clearly write the student's name and CSID number on every page.
- Always include a cover page with your Name and CSID# when sending a fax.
- Do NOT fax, email, or mail the same document multiple times or use multiple submission
methods unless explicitly told to.

Do NOT Email Confidential Information:
- Do NOT email forms to any staff in the Financial Aid Office. The email server is unencrypted
and not secure.
- DO NOT submit via email any documents that have any Social Security Numbers (SSN) on them.
- DO NOT submit via email any sensitive data documents such as but NOT LIMITED to State Driver’s License or State Identification card, Passport Card, US Passport or Military Identification etc.
- DO NOT submit via email picture of documents with any SSN or sensitive data.
- DO NOT submit via email any State or Federal Tax documents.
- Emails containing private information will be deleted. Mesa College is not liable
for any damages from emailing sensitive documents.
- Do NOT email forms to any staff in the Financial Aid Office. The email server is unencrypted

Privacy & Communication:
- To discuss your record with someone in the Financial Aid Office, a password must be on file. To create a password fill out Password Request Form for Financial Aid Students.
General Forms
You can submit completed forms in person during counter service hours, or general forms that do not require in-person submission can be submitted by;- Mail: Send completed forms and any required documents (unless specified otherwise) to:

Campus Change of Record (COR) Request
- Before Getting Started, Review the Following:
- Changing COR is time-sensitive.
- Mid-year COR requests are generally not allowed.
- If you have already been disbursed federal, state or institutional aid at CITY or MIRAMAR College for the aid year, you are likely not eligible to change your COR. If you have been awarded aid at either institution, but not received a payment, check with the Mesa Financial Aid Office before completing the COR request to ensure you are eligible.
- Your COR must align with the SDCCD Campus of your academic plan. If your major is exlusive to CITY or MIRMAR College, you are not eligible for a COR change.
- Once you change your COR to Mesa, it is irreversible for the rest of the academic
year (Fall, Spring, Summer).

To Get Started:

- Download the Campus Change of Record (COR) Request.
- Print, fill out and sign the request form.
- Submit Form Online.

Completion Indication:
- You'll know your COR has been changed when "MESA COLLEGE" appears on your student portal welcome screen.
Password Request Form for Financial Aid Students

- Print and fill out form with physical signature/manual signature/ handwritten.
- Submit Form Online
Send your request form with a visible copy [FRONT and BACK] of your valid Government Photo ID showing your signature (excluding Military ID).
Third Party Authorization-Release of Student Records
The Family Educational Rights and Privacy Act (FERPA) ensures the privacy of students' educational records, including both financial and academic information. Without the student's explicit written consent, FERPA typically restricts the sharing of this information. Hence, for any information related to your financial aid to be released, we require your authorization and password.
Only the student is allowed to complete this form. To get the release form approved, students must also submit the Password Request Form for Financial Aid Students along with a clear photocopy of a valid photo ID.
- If you haven't already done so, download and complete the Password Request form.
- Download and complete the 3rd Party Authorization Form.
- Click "Submit Form Online" to upload both forms. {select Financial Aid Form/Verification from the Forms/Applications box and complete the online submission request}
Attach clear copies of the FRONT and BACK of a valid government-issued photo ID displaying your signature (Military ID is not accepted).
2025-2026 Academic Year Forms

Please be aware that the 2025-2026 application is applicable to the Fall 2025, Spring 2026, & Summer 2026 semesters. You are encouraged to always apply as early as you are able. In the interim, you may receive automated notices indicating the application was submitted successfully, or that you may need to make necessary corrections before the school can download and process the FAFSA.
